Support Network Canada Privacy Policy — Client Intake Form (Draft) This is placeholder text written for the Client Intake Form feature specifically. It has not been reviewed by a lawyer and must be replaced with counsel-approved copy before production launch, alongside the Terms of Use. 1. What we collect: When you check in to a shelter, you may be asked to complete a Client Intake Form covering contact details, household/dependents, language needs, and a brief description of your situation, including optional questions about self-identification (e.g. Indigenous identity, disability, 2SLGBTQIA+) and gender assigned at birth. Every optional field can be left blank or answered "prefer not to answer." 2. Who can see it: Only the shelter you checked into can see your intake form — their staff use it to contact you and to prepare for your stay. It is not visible anywhere else on the platform, including to other shelters. 3. Platform administrators: A Support Network Canada administrator does not have standing access to your intake form. If an administrator believes they need to see it (for example, to help resolve an issue), they must submit a request stating why — your shelter decides whether to approve or deny that request, and can deny it. Any approved access is limited to a single view and is logged. 4. How it's used: Shelter staff use your contact details to reach out by phone or email about your stay. Nothing in your intake form is used for any other purpose, and it is never sold or shared with anyone outside the shelter you checked into (or an administrator explicitly approved by that shelter, as above). 5. Every access is logged: Every time your intake form is actually viewed — by shelter staff or, if approved, by an administrator — that view is recorded with who viewed it and when. You can ask your shelter for this history. 6. Withdrawing consent / removal: You can ask the shelter you checked into to remove your intake form at any time. Contact them directly using the details on their shelter listing. 7.
